Con un organico di 6 medici
TORRE CANNE – Sono tutte operative tutte le Unità speciali di continuità assistenziale previste al momento in provincia di Brindisi: due a Ceglie Messapica (Distretto di Francavilla Fontana), una a Torre Canne (Distretto di Fasano), due a Brindisi e San Vito dei Normanni (per il Distretto del capoluogo) e una a San Pietro Vernotico (Distretto di Mesagne). Nei prossimi giorni sarà attiva anche una settima équipe a San Donaci, sempre nel territorio di Mesagne, in una struttura messa a disposizione dal Comune.
Per le Usca, accanto alle linee guida sulla gestione ospedaliera e domiciliare dei pazienti Covid, la Asl di Brindisi ha predisposto un piano operativo che regola nel dettaglio le singole fasi dell’intervento. Le Usca sono attive tutti i giorni, dalle 8 alle 20, e ogni unità è composta da due medici, o da un medico e un infermiere, e ha in dotazione un’autovettura per raggiungere la casa dei pazienti. I locali delle unità Usca vengono puliti e sanificati ogni giorno da personale della Sanitaservice e i mezzi sono sanificati più volte nel corso di una giornata.
Le Usca intervengono per casi Covid accertati o sospetti, segnalati da medico e pediatra di base, che invia per email una scheda di segnalazione che contiene – oltre ai dati anagrafici del paziente e del medico con numeri di telefono – notizie dettagliate sulle condizioni cliniche del paziente, sulle sue comorbilità e sulla terapia in corso. Il medico di famiglia concorda con il medico Usca strategia decisionale, terapia ed eventuali analisi cliniche, esami radiologici o consulenze specialistiche. In casi urgenti il medico di base contatta direttamente via telefono le unità Usca per chiedere una visita con priorità assoluta.
“Questa collaborazione tra medico di base e Usca – spiega Donato Monopoli, segretario regionale della Fimmg, Federazione italiana dei medici di famiglia e coordinatore Udmg (Ufficio Distrettuale per la programmazione e il monitoraggio delle attività in Medicina Generale) del Distretto 2 di Fasano – è fondamentale per un’efficace gestione domiciliare dei pazienti Covid. Si è completato un percorso virtuoso che gioverà al trattamento dei casi sospetti. Un altro aspetto importante riguarda la sicurezza: i medici Usca sono dotati di tutti i dispositivi di protezione e hanno seguito un corso di vestizione e svestizione tenuto dal Servizio di Igiene e Sanità pubblica della Asl. Le sedi Usca seguono un protocollo ben preciso con un percorso pulito e uno sporco. Il pulito permette l’ingresso dei medici che si preparano per la missione. Terminata la visita al domicilio del paziente depositano il materiale monouso in un contenitore all’interno dell’autovettura, indossano nuovi dispositivi e sanificano l’auto. Rientrati in sede entrano nei locali attraverso il percorso sporco, eliminano i dpi e gli indumenti che vengono smaltiti come rifiuti speciali, si bonificano, escono dalla sede e rientrano nel percorso pulito per compilare la scheda di intervento. A Torre Canne l’organico prevede 6 medici per tre squadre, divise su due turni, che potrebbero collaborare con gli specialisti pneumologi o di altre branche”.
